    Hello Holly,

    Thanks for reaching out to Jotform Support. I checked your account and found that you have a form that has a field intended to collect usernames and passwords. This kind of activity is against Jotform's policy for the use of service. This is the reason why your account got suspended.

    We can reactivate your account provided that we have to delete the field(s) violating Jotform's terms. But we'll need your consent before we do anything. Should we proceed in deleting the fields involved?

    As soon as we hear back from you we'll help you more with this.

    Thanks for getting back to us and giving us the authority to remove the field affected. I have done this and I have re-enabled your account. You may now enjoy using your account and your forms.

    Generally, what would appear to be sensitive or even financial that can take over another person's property is prohibited from being collected here. An example is the collection of credit card information, collection of social security numbers without any purpose. Or in your case, the collection of a combination of usernames and passwords.
