I am working on training document where we sign off on individual work and like to discuss with a professional
I want to be able to update and make note and make it more of a live document

I have my form

I want individual to complete where they feel they with each selection
Then I want my manager to review and make comment for each question
and sign off if they feel the client is ready to move on to other things per question

Hi Jason,
Thanks for patiently waiting. I've got an update on your request. I've checked out your Mechanics of Faith Skills Assessment and I found on the clone form that you have set up the approval flow. You can use the approval flow to achieve your requirements. But, you can also use the edit submission form filed in your email notification and the show hide conditional logic. You will need to add the short text field under each question and hide it conditionally. You will also need to add the get form page URL widget and hide it. Let me show you how to do it:
- In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
- In the menu on the left side of the page, click on Emails.
- To edit it, click on the Pencil icon on the right side of the Notification Email.
- Then, under the Email content, click on the + Add Form Fields, and choose Edit Link.
- Click on the Advanced tab, uncheck the option of when the submission is edited.
- Scroll down and click on the Save button. That's it.
Now, let's add the Comment Short text field under each question and hide it, then add the widget and the condition.
- Open your form in Form Builder, and in the orange navigation bar at the top of the screen, click on Add Form Element.
- Select Short text field, drag and drop it under each question.
- Click on the black gear icon for the short text field.
- Choose the Advacned tab and turn the toggle on to hide the field.
- Select the widget option and search for the Get Form Page URL; drag and drop it on your form.
- This widget will be invisible on your form but visible on the submission table.
- Scroll down to your form at the bottom and turn it Off Jotform Sign Automation by clicking on the green Pen icon.
- Turn the toggle off for sign automation.
Add the following condition using the Show/Hide conditional logic. Don't forget to add all the comment short text fields you created under each question. That's it.
