Good afternoon, I have set up an account and conducted a Zoom meeting with Janelle Maffucci concerning my account. I was advised to reach out to support because we conducted a trial of scanning a QR Code, and I was supposed to receive the form via email once a customer completed it, but I haven't received any emails yet. Please assist. Janelle Maffucci mentioned there might have been a glitch, but before proceeding, I would like to check with Support first.

Thanks for reaching out to Jotform Support. I’m sorry you’re having trouble with this. May I ask if you are referring to this form? If yes, then I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on. If this is not the form that you are referring to, can you share the Form URL so we can help you better? Here's how to do it:
- In Form Builder, click on the Publish tab in the orange navigation bar at the top of the page.
- Click on the Quick Share tab on the left side of the page.
- Click the Copy Link button on the Quick Share window.

Thanks for getting back to us. I'm sorry, but we don’t currently offer live phone support. Although we do offer Live Chat Support, there are a limited number of agents working in each time zone. So, sometimes, it gets disabled, and you can't see it on our pages. When it is available, you'll see a pop-up if you're logged into your account on most of the pages, like Form Builder, Jotform Tables, Jotform Inbox, and so on, like you see in the lower-right corner of the image below:
If you don't see that option on the page, and you have a question, just reply to this thread, or you can create a Forum Thread from the Contact Us page. We’ll be happy to help. Let me show you how to create a Support Ticket:
- On your My Forms page, click on Support on the top-right side of the dark-blue navigation bar at the top of your screen.
- In the menu that opens, click on Contact Us.
- Enter a title for your question in the box below ‘How can we help?’
- In the box below the title, enter some text that explains the situation.
- Now, just click on Post Question on the bottom-right side of your screen, and you're good to go.
You can also:
- Add the link to your form or to the website that you embedded the form into by pasting that into the box under the Add a Link section.
- Click on Choose File under the Upload a Screenshot section to upload a screenshot of the issue.
- Tick Make Private to prevent other users from seeing your question or answer. We recommend that you tick it if the content includes sensitive information. If not, you can leave it unticked so other Jotform users can benefit from the answer.
Once we hear back from you, we'll be able to help you with a solution.
