Account suspended: Potential Fake service

    Hi Mary,

    Thanks for reaching out to Jotform Support. As I checked, your account has been suspended and tagged as a potential Fake Service Offer. We understand that this may not be the case, but we have to follow the general rule in order to be in compliance with our Terms. If you wish to reactivate your account you will need to provide the following information.

    • The URL where your form is used.
    • Your website or proof of affiliation to a legitimate institution.

    If you have an email under your organization, please email us with all of these to reviewers@jotform.com using "thread #14733801" as your email subject. One of our colleagues will get back to you about the status of your account.
