why, when I try to preview and fill out my form to check the conditions, do I receive a "form disabled" message?

Due to you had exceeded monthly payment submission limit your accunt status is 'OVERLIMIT' now. Free accounts can have receive 10 payment submissions per month. Your submission quotas resets at the 1st day of each month. During this overlimit time not just this particular form but all your forms can not receive submissions. In case of you want to check your qouta status you can see it on the left side of your "My Forms" page. If you do not want your forms to be interrupted due to 'overlimit' you can upgrade your account during this time. Our premium accounts are affordable at $9.95/month with 1000 payment submissions per month. You can upgrade or downgrade when you need it. When you downgrade you do not lose your forms or submissions.
