Form: What does receive three payments mean?

With the free form option it says "receive three payments" as part of the package. What does that mean exactly? Three free payments received? Or three payments per month? If three payments free, then what is the charge for those above three per month?

The "receive three payments" is the total payment you can receive for the month.

If you use a Payment Form or use the Payment Integration to receive donations or payments, you are limited to receive a maximum of three payments. When you receive more than three, your Forms with Payment Integration would be disabled.
