Configurable List widgt not working sometimes

I have a form with two configurable lists. Both are required fields and will show based on their answer to the questions above each. Sometimes they both work/show. Sometimes one will show when it should and sometimes neither will show. I'm assuming they are not 'showing' since they are required fields and the form is being submitted without an answer. I am embedding this form HERE. The scenarios below are on the same forms with the same questions. I cannot recreate to fix. It always works for me. fyi the custom CSS used in the CL is from JotForm support.

Example one. BOTH WORKED

Example two. #1 Will show if populated and 'None' is not selected...Worked.  #2 They answered yes so should have had to fill in a configurable list for question '2'. They did not....assuming it did not display.

Example three. #1 Will show if populated or 'None' selected...Did not work.  #2 They answered yes so should have had to fill in a configurable list in the next question. Neither worked. Assuming nether displayed.

If yes, please try to disable the Continue Forms Later feature.

After that, try to change the Clear Hidden Field Values and set it to Clear On Being Hidden option.
