I see that you are currently using a Free Account. Basically what has happened is you've reached your form quota for the month and what this means is that our system has disabled your forms because of this.
This leaves you with two options:
(1) You can wait for your Monthly Reset (12/03/12)
(2) You can Upgrade (http://www.jotform.com/pricing)
