Adding form inside a fixed div

I have a form I'm trying to insert into a "Fixed full screen div".  I want it to pop up as a modal window after clicking a button.  The div is set to Fixed: Full.  Then the form is placed inside.  But when I publish the div shows but the form is missing.  I have all of the content set to z-index: 99999.  So it should show above all other content.

What am I missing?
