I'm using the lightbox embed code and its giving me a 404 error when clicking the link to launch the lightbox. It does this even in the Test LightBox page in the JotForm admin site. Any ideas on what I could be doing wrong? I thought it was going to be as easy as copy and paste the code that is provided, but it doesn't seem to be working for me.Page URL:

I can see that you are embedding two (2) different lightbox codes into that page given and I suspect they are conflicting;
Please try removing the first or any of them and leave only once (1) embed codes and see if that helps you view the form.

If that is the case, then why do I get the same error when testing on jotform.com? I removed one of the links and I'm still gettting a 404.

It is simpy because you have overlimited your free limits for this month. This is the reason why the form isn't displaying anymore and why it shows that error. Your forms are temporarily disabled due to this reason. Upgrading to one of our paid plans should help. JotForm · Pricing. However, if you don't like to upgrade you have to wait until your monthly submissions resets to zero which will happen every first day of the following month. For example, next reset will be on December 01, 2012.
