Some iOS users are seeing blank video

I have a form that is consumed mainly from mobile devices, on Android there is no a problem, but in iOS a blank video appear the the form is opened 

Hi, I tested your form in an Iphone 8 using Safari as well in an iPhone X using the same browser and I was able to load your form with no issues.

This seems to be a problem with conflicts between scripts of your page and the form, if you're embedding the form in another page, could you please provide the link for us to inspect it further? You mentioned that your form gets replaced by an empty video player in Apple mobile devices, could you tell us in which one you tested so we can reproduce the issue?

Hello, thank you again for the response. It still doesn't work but its only this form, I have another (https://form.jotform.co/81554360619863) that get presented in same way and it works perfectly.

Sorry, I wasn't able to follow the steps in your video. After the first page "Escribe tu nombre completo", what did you press to pull the video screen?

The blank video doesn't prompt on my end after redirected to the first page of your form. Would you mind providing us with more details like exact iOS device, iOS version, browser type and its version?

Hello, when I enter to the form and navigate to "Escribe tu nombre completo" I wait couple of seconds and the video appears.

The device is:

Iphone 6 Plus with IOS 11.4.

The problem ocurrs on native iOS WebView inside the an app. 

As I said, the problem is only with this form, I tried to duplicate the form and behave the same way.

I was reviewing what the WebView structure and how it works. It seems that its ability to take a picture using the device’s camera. You can review the description here https://developer.telerik.com/featured/what-is-a-webview/.

It was probably triggered by the app before it was redirected to Jotform. Just want to confirm if the video prompts if you haven't proceed to the first page "Escribe tu nombre completo"? 

Kindly confirm if video still loads even if you haven't click the "Iniciar" button.

Hello guys, I found the solution, the "Upload file" element was causing the issue. I removed it from the form and all is working as expected
