Incomplete values error: Cannot submit form

When I fill out the form emailed to me it says values incomplete and will not let me submit it.  https://form.jotform.com/81693362141152

Did the error mention which fields were affected?

Also please tell us if this error occurs on your stand alone form ( via direct link ). Or have you embedded this form in a website somewhere?
