I followed all the instruction and cant get it to display, please help me with this.

Hi,

I am sure that you forgot something to follow. The guide is so detailed. Can you give us the URL of the form so we can give you the codes to paste with?

EDIT:

I would suggest to please redo everything and check the guide from first step to last.
