    Hi Jotform,

    I messed up, sorry about that. I wasn't aware that we couldn't ask for passwords or sensitive info on Jotform and my account was suspended afterward. I looked up what the error message meant and saw that it was due to that. Can you please re-enable the account and I will make sure that this doesn't happen again? Thank you very much.

    Upon review of your account it was found the collection of login credentials.

    We have cleansed the form and reactivated your account, relabeling the form with a different name: in order for it not to draw unwanted attention.

    Your account is active and your form available. We whitelisted your IP so you can login to your account.
