I complete the form, attach the document then click submit. A second CAPTCHA code appear, which I fill in, but the page just goes blank. When I check my Google Drive account (integrated properly) no submission is visible.

Stuart, kindly try to remove the current captcha on the form and re-insert it. I just did that on a clone form and it did the trick.
