How to get field to show on form?

I have created a form, however one of the fields is not showing up when the form is published.  Missing field is "Team Played On"

How do I fix this?

The field that you are referring to is associated with a condition. You need to delete the condition in order for the field to show. Here's how:

In the Form Builder, select the "Team Played on" field and then click the "Conditions" button.

After that, select the condition and then click the "Options" icon. Click the "Delete" icon to remove the condition.
