The link to my form is no longer working

Hi I have a form that has been working well. However for some reason the link has stopped working. Could you please look into this for me as I need it functioning asap.

As we check the form using the direct link https://www.jotform.co/form/81829530904864 it is working fine. Since the form is enabled with the custom URL, it seems to be working fine. Please reset the custom URL from the Publish wizard and the form should be working fine.
