I am trying to create a form for an account setup and my account is suspended

Hello. I am trying to create a form for an account setup and my account is suspended and I cannot access any forms. 

You have been auto suspended by our system due to your form that is created to ask for username and a password. Please note that we don't have a direct database integration that allows you to create an account registration form. We consider this kind of form as phishing which is against our terms of use.

I have reactivated your account, but I have deleted the fields in the form. Please refrain from adding the same fields again. Otherwise, we might put your account to permanent suspension.
