can I upload an existing form?

What I'm hoping to be able to do is upload a form I've created.  Then have it published as a 'fill in' form to be used monthly by several groups.

If the form is a web form in HTML format, it is possible to import the form using the web page URL:

If the form is in another format like a word doc or PDF, we do not currently have a tool to allow for importing that type of form.
