Form insertion into my website?

    I need the following contact form to be used as a Contact page for my website:

    How do I find the HTML code and how do I insert this into my website?  I have no clue with this stuff.  THanks.

    Hello  HarmonyWreaths.  There are a number of ways how to get your contact form to be placed on to your website.

    Primarily, following this user guide will provide you the javascript source code which you can get and place into your web page.

    Example you were able to get this script source:
    <script type="text/javascript" scr="http://www.jotform.us/form/23350712661144"></script>

    Next, edit your website's Contact page and insert the code to where it should appear, save your page and preview it using your browser.

     

    There are also a number of ways how to get your form's source. The mostly common are getting the iframe and also the HTML source of your form.  You can also read on this user guide to know more.
