Forms not submitting correctly

All my submitted forms come in blank from email and are not registering in my jot forms. This has happened on more than 1 occassion

I have tested a cloned version of your form and I was redirected to the PayPal page after submission.

May we know if the submissions are found in the incomplete payments?
