I would like to have a 2-step form where a prospect enters their initial very basic information, then on the 2nd page it lets them know of an additional gift and they just need to put their email address in the 2nd form to get the gift (a series of recipes).
On the first form we do not have their email address as they are requesting a printed report to be physically mailed to them. We found that our acceptance rate increased when their was NO email on the 1st opt-in...so we would like to have the thank you page populate a new form that lets them enter their email. This is basically a one-time-offer (OTO) type of page.
I have tried creating an additional form, then embedding it in the html of the thank you page with no success.

I checked this form's Thank you page, and I think you had it wrong. You cannot paste the form's source code into the TY page. It will only get parsed and cleared.
May I ask then if you can use the Page break field on your form?
Make it a 2 page form.. the email field is on the 2nd page.
