How to unsuspend account?

I received and e-mail from Jot Form saying my account has been suspended with this link

When I click on the link it says "page not available"

Please advise.

Your account danhowelltravel was downgraded to Free and is now over the limit. This usually happens when you did not subscribe to your paid plan on time. Or your recurring billing per month did not occur.

To fix this, please log in and resubscribe to the plan of your choice. I have reactivated your account so you can now upgrade to the paid plan you were on before.
