Create a lead form that a user can click on to open

Hello,

Is it possible to create a simple lead form (Name, Phone, Email) that will sit hidden in the bottom right of my website with only a tab visible that says Quick Quote or Questions?

Once the user clicks the tab text, the lead form will pop up with the required fields (above) visible.
