How can I create a Customer Survey form to help collect online reviews?

     Hello everyone, I am new to JotForm and I will like to create a simple customer satisfaction survey through JotForm survey maker. The survey shall allow prior customers to select between 1 and 5 stars.

    If they select 3 or less stars they will get a popup box for them to include a comment on how we can improve our services. Now, if they click on 4 or 5 stars they will get another popup including links to Google, Facebook and Yelp for them to leave a review on any platform they choose.

    I became familiar with the JotForm platform and already created a nice online form, but this seems to be at a higher level of experience. I will greatly appreciate the help!

    I would suggest creating the form with the appropriate fields. Add the comment area in the form as well as the links to the review sites. 

    Now, you will need to configure a condition in the form show the comment box when 3, 2 or 1 is selected in the star selection. Do the same to show the review sites when 4 or 5 stars are selected.
