Can only open my form on a computer. Cannot access from an iPhone or iPad.

Your form stand alone version is fine. Note that JotForms Form can be viewed properly on every mobile devices. I suspect the problem lies on the facebook page. As far as I know there's a known issue on facebook pages when viewed on mobile devices, it will display "page request not found", is this the problem you are referring to? I think the best way to have it resolved is to address your issue to Facebook Dev Team so they can provide an appropriate solution regarding this. Or, tweaking up your facebook app to allow mobile view might help.
Another possible solution is to switch your view to "Desktop" instead of "Mobile" views when browsing your page via Mobile.

I believe that my colleague is right about the mobile version of the Facebook. When someone access the site from iPhone, the Facebook is redirecting to the mobile version, and that mobile version can not see some page tabs with content.
Your users will need to switch to the Facebook Desktop version on iPhone, and then they will be able to see everything on your Facebook page.
I think that the iPad users are not being redirected to the mobile version.

Is there a link I can direct my clients to that bypasses Facebook? The stand-alone you mentioned?
