The form I created is not working

My web design team used you guys for creating a form on https://www.ftek.com.au/application-form however it has stopped working and they have advised me to contact you to find out why

I checked the website you provided and I can see the form is displaying properly. Here's a screenshot:

Is the issue still persists on your end? If yes, please provide a screenshot so we can further check it. Also, please try opening the website using a different internet browser for testing purposes.
