    Hi,


    I was able to re-login, but had not read your email that setting up any form to collect credit card details is not allowed. As I did not read the email forst I was re-creating the form and was again locked out. Can you please reactivate and I will no longer create this credit card authorization form on JotForm.

    I have removed the credit card information fields in your form and lifted your suspension.

    Please read our -Terms-of-Use to avoid being suspended again.
