My form was disabled and I can't log in

I joined last night and created a form. Today I am unable to log-in and my form has been disabled. Can you please help?

Upon review of your account it was found form: http://www.jotform.us/form/82328793202154 was collecting ssn# and was offering real estate assistance without a valid banking or credit union domain.

Please be advised that these type of customer's information collection is not allowed to individuals that are not in the business. If you are, we need you to furnish proof of business or affiliation to a banking or credit union institution to support@jotform.com. Otherwise, you have to remove forms of this type. Our phishing detector system will flag the forms and suspend them when in violation of terms of use. We cleansed the form and re-activated your account.
