Multiple places where initials are required on a form

Hello,

We have a form that clients complete to pre-register for our GED program.

At the bottom of the form, clients are needing to initial multiple times, and I'm not certain why but I would like to cnage this.

Any assistance that can be provided is appreciated.

I am not sure if I have understood your requirement correctly. I did check the form you have created and you seems to have already added several fields for initials. Please check the screenshot below:
