I have had multiple clients have difficulty selecting the date.
My most recent client said: "Also, the date in this questionnaire seems to be screwed up, even when selecting a date from the calendar it says I have the "wrong format."
This happens quite often. Please look into.

I don't see the date field added to your form however I cloned your form and added a date field to test. The Date picker field works absolutely fine. Can you help us with the exact issue? What trouble your clients are facing when they use the date field?
