How do I get the form back?

    Hi, 

    I have a form under the free plan and went over the size limit.  I just logged in to upgrade to keep my form going and it's been deleted.  I'm going on vacation in 3 hours and this will prevent my website from working while I'm away.  How do I get the form back?
