How do I make my website form look like your form preview of it?

My mobile form is not looking like your site's preview of the same form.

Please see the attached screenshot comparison.

I tried, and removed your mobile responsive widget. It made no
difference.

What changes do I need to make, to match your preview?

The form adapts to your website, since the website itself is wide, the form becomes wide too.
