    contact form formatting disappeared from my website!

    Hello, I'm using your envelope style contact form, and have been for a few weeks, but the formatting of it just disappeared from my website! I logged in to Jot Form, and the formatting has disappeared from my Jot Form templates, too—it just has the basic fields. Please help me get it back.

    Hi,

    Thanks for reporting such case. I have now forwarded this issue to our developers as I could also replicate such.

    The form function is not affected by they way. It still works, it is just the background image URL has been broken.

    I am looking at the screenshot you posted and it looks fine. Exactly what is missing on your form?

    Hello, the screenshot was of the way the form was supposed to be, not of
    the way it was actually publishing on the website. However, for some
    reason, it is now back to normal on my website. You all must have fixed
    something.
