My account has been suspended

    Can you please let me know why my account has been suspended?  My customers can no longer use the form.

    Upon review of your account I found that it was suspended because of forms: https://www.jotform.com/80074369379164https://www.jotform.com/70067020937149 and https://www.jotform.com/50650803080143 are requesting ID Upload.

    Online Form Builders, including JotForm, are sometimes being used for identity theft and we go to great lengths to prevent this. That is why collecting these types of data is considered as phishing activity, as stated in our Terms of Use.

    If your purpose is not to collect identification numbers from the general public, but from your own customers, then you need to comply with two things.

    1. Add the Terms & Conditions Widget  for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    However, we do not allow uploads of any sensitive personal information like passports, visas,social security card, driver's license, credit/debit cards, bank statements, voided cheques and photo IDs.

    Upload buttons are not designed to protect sensitive information, so we suggest to refrain from using them with this type of data. In case of a security breach they are not going to secure the information at the servers.

    I have deleted those upload buttons in order to reactivate your account.

    We collect our customer IDs because we operate a studio rental that needs to validate their ID when they sign up to make sure they are over 18 and check their names. I can make a change to the forms to not upload any ids. I will have them email me separately. Can you reactivated my account so I can make a change?
