I have been checking your form and I could not see the sales person signature field appearing when I was filling the form as a customer. I can see you have the autofill feature enabled so it may be that the previous selections are saved so try using your form in incognito mode.
If the problem persists, let us know.

No luck. I tried after clearing the cache and going Incognito.
