Why is my account suspended?

I did check your account and found that it is currently suspended. It seems you have form "[Imported Form] Rifa Octubre – Xpressate" in your account which is suspected to be used for phishing activity because of the Username / Password field you have added in form. Please check the screenshot below:

I have deleted the suspected form and activated your account again.

Do try to sign in to your JotForm account and get back to us if you have any trouble.
