How to finish customizing model release?

Hi, I'm using a model release form pdf, I customized your sample, but at the end there's a box to check in order to affirm acceptance of the terms and I can't figure out how to customize it, it still says "I have read and understand the terms of this model release with Unforgettable Photography by Jenn." How do I change it so it has my name, and not "unforgettable photography by jenn?"
