We are having an issue with users receiving an error when submitting pictures with the form.

The take photo widget is quite hit or miss on mobile depending on various browser permissions and settings. For best usage, I would recommend Chrome Mobile for both iPhone and Android. Either that or take the photo on the device and then upload using a regular file upload field. The widget itself does appear to be functional.

weird thing is that we had 41 straight submissions without an issue until today. We recently upgraded our account to Bronze and since have not been able to load the pictures. I have tried through taking a picture or selecting from library and the error is received for both. Having all users submit through Chrome is not feasible for us

When it comes to mobile devices, you can use a regular upload field as well:
Since when you're using it on mobile, it will offer to take the photo with a camera:
