Hello derekdo.  You can get your CSV file from your submissions list or create a CSV report of your form's submissions and then store it in your hard drive.  You can find the instructions on how to do this on this user guide page.

    Import to Outlook

Before you can import your contacts to Hotmail, you should import your contacts to Outlook first.

To import contacts from a CSV file or from Excel into Outlook:

    1. Select File | Import and Export... from the menu in Outlook.
    2. Make sure Import from another program or file is highlighted.
    3. Click Next >.
    4. Now make sure Comma Separated Values (Windows) is selected.
    5. Click Next >.
    6. Use the Browse ... button the select the desired file.
    7. Typically, choose Do not import duplicate items.
    8. Click Next >.
    9. Select the Outlook folder you want to import the contacts to. This will usually be your Contacts folder.
    10. Click Next >.
    11. Click Map Custom Fields ....
    12. Make sure all columns from the CSV file are mapped to the desired Outlook address book fields.
12.1 You can create new mappings by dragging the column title to the desired field.
12.2 Any previous mapping of the same column will be replaced with the new.
    13. Click OK.
    14. Now click Finish.

    Extract CSV data from Outlook

Next, get the CSV file from Outlook.

    1. Select File | Import and Export... from the menu.
    2. Make sure Export to a file is highlighted.
    3. Click Next >.
    4. Now make sure Comma Separated Values (Windows) is selected.
    5. Click Next > again.
    6. Highlight the Contacts folder.
    7. Click Next >.
    8. Use the Browse... button to specify a location and file name for the exported contacts. Something like "Outlook.csv" or "ol-contacts.csv" on your Desktop should work fine.
    9. Click Next > (once more).
    10. Now click Finish.

    Import to Hotmail

Once you have the data from Outlook, you can login to your Hotmail account > Scroll down until you see Contacts on the lower-left part of the page.

    Next, click Import from file.

Next, select the file and then click Import Contacts
