Create a field with sub-categories

Hi, is there a widget that will allow someone to select major categories, then subcategories then enter a number.

Then inside those categories have check boxes and enter quantities?

Something like this:

> Vodka

  > Grey Goose

    > 3 bottles

We have the Dynamic Dropdown widget, however, the sub-categories for this widget will also be a drop-down. May I suggest that you check the Configurable List widget and see if it will meet your requirements, please?

Also, how would you like to consider have a regular checkbox, dropdown, and/or textbox fields instead that will show up conditionally based on another field's selection or answer?
