account says suspended, can't log in

Upon review of your account I found that it was suspended because of forms: http://www.jotform.us/form/72834848362163 and http://www.jotform.us/form/72834727462160 are requesting login credentials (username/email/password) and social security numbers.

-The collection of login credentials, infringes our Terms of Use, as this is only allowed through encryption along database administration; and our forms do not provide this kind of service, those fields will be removed in order to avoid non-compliance with our Terms of use. 

-If you need to collect social security numbers  then you need to comply with two things.

1. Add the Terms & Conditions Widget for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

I have deleted the infringing fields mentioned previously and reactivated your account.
