Auto suspended account

    I had a google form that I was loading up to edit. Apparently it was not allowed, something with credit card? I am not positive,  but it suspended the account. How do I gain access to our account again ?

    Upon the review of your account I found that it was automatically suspended (autosuspended) by our Phishing Detector Tool because of form: http://www.jotform.us/form/82837342984166 requesting credit card numbers and banking information on plain field texts.

    -Due to our PCI certification, we are required to deny the collection of credit card numbers on regular forms.

    However, we have other PCI compliant ways to do this, all you need to do is to choose any of the payment processors available in our platform in order to integrate in your forms, you still won't be able to collect credit card numbers directly, though.

    -Regarding the collection of banking information  it depends on why you want to collect your customers banking information:

    If you are using the form in order to create a direct deposit or a refund you need to add our two mandatory widgets, that allows your customers to authorize the collection of their banking information:

    1. Add the Terms & Conditions Widget for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    If you have created this form in order to collect payments I highly recommend you to use one of our "Payment Processors" in order to avoid non compliance.

    I have deleted the infringing fields and reactivated your account.
