We are considering integrating your services into web sites we support for a nationwide collection of dentists for those forms on the site asking for HIPAA covered information. If we go that route, here are some questions:
- will we be able to set up the forms to e-mail their information to a recipient address at the dental practice?
- if the answer to the last question is yes - can we pass in the recipiant e-mail address(es) as part of invoking your service on the web page? I presume the technical implementation is iFrame - so can the "send the results to" information be passed in as part of the iframe invocation?

Notification can be set in your form, but if that is protected, it will not be visible.

I am not sure what you mean about notification - so is the e-mail that gets sent just a "someone filled out this form" notification - or can it contain the data provided in the form?
To your question - I am working to understand what is possible in defining the "to" address for the e-mail the form triggers. For example, say we build a form in your system. Let's say we have a web page for Dr. X and a web page for Dr. Y - but we want to use and deploy the same form for each - but the notifications or data needs to go to different doctor offices/e-mail addresses. Can we set your technology so we can dynamically supply the destination e-mail from the individual pages as part of invoking the otherwise consistent form, and that data that we supply changes the e-mail destination and perhaps gets stored in your system as part of the form fill?

While on the HIPAA, you can opt to show the data submitted in the form in the email or not. The following guide should help you with it:
