Form Settings: Change the layout option

When I clone form "wickham pharmacy" is changes from single question/page to all questions on a single page.

How do I claim a form that preserves design and format?

When you are cloning form, you may use either of the following options as per your needs.

Also, if on cloning the layout of the form is changed then you may change the layout from the Form Settings.
