My account has been suspended and I'm not quite sure why. Please respond asap I have people waiting to use

    Looking for a resolution to my application form issue. My account has been suspended temporarily for some reason. Request immediately support to rectify this situation or please inform me of the necessary steps to correct.

    I see that the account is suspended due to the file upload field to collect the driver's license which is not allowed with JotForm. Please note that it is not allowed to create login forms or collect sensitive information like Credit card numbers, ID proofs to upload, etc through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing.

    In order to activate your account, it is required for you to agree with our terms of usage and agree to delete the guilty fields on the form. Please confirm so that we can delete the guilty fields on the form and activate your account.
