I believe that I have followed the process given to the "T" but it still doesn't work.

Hi Jim, I apologize in advance but currently we do not provide phone support at this point in time. I am sorry to hear that this is giving you trouble. I've tested your form and I was able to recieve submissions and it appears to be setup and working properly.
