I got a bizarre survey response from Jotform

I think my personal information is at risk. I got an email titled "Flower Essence Therapy- Client Intake Form" which I know nothing about. There was personal information on it. Please provide an update of who this company is and how they're working with you.

You can request them to remove your information from their list; they have the contact information here https://www.wildflowerclinic.com/contact/. Otherwise, like if they don't respond, let us know, and we'll be happy to delete it for you.
