There are a few fields that won't show up in the email that is forwarded to us once a form is filled out. Are there settings that need to be adjusted?

Sorry for the inconvenience that may have caused. Are the following fields on your form are all set to required or not? If not, there's a possibly that the user didn't filled up those fields. But if yes, may we ask your form url for us to investigate the said issue?
