Users get the error, "pci.jotform.com took too long to respond." after the browser hangs when users hit the submit button.
What's weird is that while there is a checkout, I have a custom "pay by check" option, which has worked on all our forms, but is not working on this form.

I have just tried making a test submission to your form and the submission went through.
Can you please test the form again and see if the issue persists?
